The Community Living Supports Supervisor is a key associate at Hope Network whose primary responsibilities include: Provide leadership, support, and guidance to all CLS Program Staff to ensure the health and vitality of the consumers we serve are consistent with the mission, vision, and values of Hope Network.
This position will encourage collaboration with clinical and other support staff internal and external to ensure all persons served are provided with consistent comprehensive treatment plans in the least restrictive environment.
Advocates and supports community integration efforts and the building of natural supports and independance.
The CLS Supervisor will be responsible for program supervision and collaborating with the community to assist with transition and innovation of service delivery.

1.
Regular and predictable attendance is an essential requirement of this position.

Provides council and support to the Program Staff in all areas of employee relations in coordination with Talent Management (coaching, discipline, performance improvement, training, and clinical provisions).

Utilize the Recipient Rights process and standards of care expectations as a means to support and enhance quality care for persons served through Community Living Supports.
4.
Support CLS staff and person served by providing services consistent with Trauma informed care and MANDT.
5.
Reviews outcomes and monitors referrals to ensure program meets or exceeds agency and funder expectations.
6.
Build and maintain effective working relationships with community stakeholders.
7.
Monitors program to ensure maximum operational efficiencies.
8.
Process financial reports and make decisions that meet or exceed budgetary goals.
9.
Market and educate community and private sectors to enhance community support and involvement.
10.
Evaluate current program opportunities while researching potential areas of CLS program expansion.
11.
Plays and active role in intake process and developing communications.
Coordinates services with internal departments, the individual, and their families/guardians to ensure the best possible service.
12.
Other departmental duties as assigned.
Educational / Talent Requirements: 1.
Bachelor’s degree from an accredited University or College in a related field.
Experience may be considered in lieu of degree.
2.
Excellent professional skills including; communication, listening, negotiating, critical thinking, and effective problem solving.
3.
Capable of coordinating multiple priorities and has the ability to change tasks as needed to meet the needs of the program and/or agency.
4.
Communicates information and ideas clearly and effectively both in oral and written formats.
5.
Proficient in all Microsoft Office Applications 6.
Proficient in the use of electronic medical records.
7.
Demonstrated ability to work with minimal supervision.
Work Experience Requirements: 1.
Management experience with demonstrated ability to lead effective teams.
2.
Experience with establishing and maintaining positive external stakeholder relationships such as case managers, funding sources, clinicians, etc.
3.
5 years’ experience working with people with disabilities in a community based setting with a successful track record is preferred.
