```Duties```
- Provide direct patient care, including administering medications, performing physical examinations, and assisting with activities of daily living.
- Monitor and document patient vital signs, symptoms, and changes in condition.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Assist with the coordination of patient appointments and referrals.
- Conduct health coaching and education to promote wellness and disease prevention.
- Perform various nursing procedures, such as wound care, catheter care, and airway management.
- Administer medications and treatments as prescribed by healthcare providers.
- Maintain accurate and up-to-date medical records.
```Experience```
-At least two years of LTC, MS, or Home Health experience.
-BLS (AHA) - Required.
-Covid Vaccine Required - _Religious/Medical Exemptions allowed_
